AMPM Care handles personal and sensitive health information with care. This policy explains what we collect, why, how we protect it, and the choices you have.

Who we are

AMPM Care ([registered entity name], ABN [ABN]) is an Australian NDIS-registered mental-health service. We are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs), and, as a health-service provider, by the health-records obligations that apply in our state or territory.

The information we collect

We only collect information we genuinely need to provide and support your care.

Personal information

  • Your name, contact details and date of birth.
  • NDIS details where relevant (participant number, plan or plan-manager details).
  • The details of a person making a referral on your behalf (a GP, support coordinator, family member or carer).

Sensitive (health) information

To provide therapy and supervision we may collect health information: for example, presenting concerns, relevant history, and notes created during your care. Health information is "sensitive information" under the Privacy Act and is given a higher level of protection. We collect it with your consent, or the consent of the person lawfully authorised to provide it.

How we collect it

  • Directly from you: by phone, email, our website enquiry and referral forms, or in session.
  • From a referrer with the appropriate consent (e.g. a GP or coordinator).

Please don't include detailed clinical history or crisis information in our website forms; they're for first contact only. We'll gather what's needed securely once we're in touch.

How we use it

  • To respond to your enquiry, arrange an appointment, or process a referral.
  • To provide therapy, clinical supervision and related support.
  • For necessary administration, including NDIS claiming and our own quality and insurance obligations.

We do not sell your information, and we do not use it for unrelated marketing.

When we disclose it

We only share your information where it's necessary for your care or required by law, including:

  • With practitioners directly involved in your care.
  • With the NDIS, your plan manager or support coordinator, where this is part of your funding or care and you've consented.
  • Where we're permitted or required by law (for example, a serious risk to someone's safety, or a lawful request).

How we store and protect it

Your information is stored securely, and access is limited to people who need it to support your care. Website enquiry and referral forms are transmitted to us via a third-party form-delivery service ([form provider, e.g. Web3Forms]) and delivered to our practice email; we hold those records under this policy. We keep health records for the minimum period required by law and dispose of information securely when it's no longer needed.

Accessing and correcting your information

You can ask to see the personal information we hold about you, and ask us to correct it if it's inaccurate. Contact us using the details below and we'll respond within a reasonable time. In limited circumstances the law allows us to decline access. If so, we'll explain why in writing.

Making a complaint

If you have a privacy concern, please contact us first. We take it seriously and will work with you to resolve it. If you're not satisfied, you can contact the Office of the Australian Information Commissioner at oaic.gov.au, or the relevant health-complaints body in your state or territory.
